Obituary of Warren Collinsworth

COLLINSWORTH, Warren Scott age 61 of West Chester, Ohio and formerly of Lebanon, Ohio passed unexpectedly Thursday evening, October 18, 2012 at Miami Valley Hospital in Dayton, Ohio with his family by his side. Born March 9, 1951 in Cincinnati, Ohio, Warren was the son of Ralph and Betty (Blythe) Collinsworth and was a loving husband, father and grandfather. Warren was a 1969 graduate of Lebanon High School, enjoyed the Miami Dolphins and Marlins and spending time with his family. Preceded in death by his parents and brother Darryl "Turtle" Collinsworth, Warren leaves behind his loving and devoted wife of fifteen years Mary (Rought) Collinsworth of West Chester, Ohio; sons Michael Pickens Collinsworth and Scott Barr Collinsworth both of West Chester, Ohio; brothers Terry Collinsworth of Florida and Ralph Collinsworth of Lebanon, Ohio; sisters Pam Blazer of Hillsboro, Ohio, Donna Barnes, Jana Wells and Gayla Collinsworth all of Lebanon, Ohio, Roberta McAfee of Monroe, Ohio and Rebecca Zinn of Florida; grand children Amanda Marie Barr and Jaryd Scott Barr and great-grandson Devon and many other family members and friends. Friends may call on the family Sunday, October 21, 2012 from 5:00pm until 7:00pm at the Stine Kilburn Funeral Home. A memorial service will be held at the funeral home at 7:00pm.
