Obituary of John R. Pacak Jr.

PACAK Jr., John Richard age 70 of Lebanon, Ohio passed peacefully at Dayton VA Medical Center after a courageous battle with his family by his side. Born September 30, 1947 in Youngstown, Ohio. John was the son of the late John Sr. and Mary (nee Hornak) Pacak. He was a loving father, grandfather and uncle. John enjoyed hunting, fishing, spending time with his grandson and watching television. Preceded in death by his parents, sister Janice Chrissman and grandson Diondre Pacak. John leaves behind his loving daughter Kristie Pacak (Kris Loggins) of Lebanon, Ohio; brother William (Mariann) Pacak of Campbell, Ohio; nieces Maureen, Charlene, Christine and Marcia; grandchildren Izayah and Annaliese; granddaughter-in-law Mi'kia; great-granddaughter Dre'Yana; beloved dog  Ali and many other family members and friends.
